you are doing so with the understanding that you are privately
paying for these tests. MyHealthResults does not bill Medicaid, Medicare, or
private insurance. If you are covered by an authorized corporate wellness
program, authorized practitioner, or authorized wellness partner, please enter
your promotional code when you order.
Where do I get my tests?
MyHealthResults.com has accounts with certified laboratories nation wide
and you will be direct to the certified lab closest to your home. When you
purchase a Smart Health Profile, it includes your online Smart Health Report™.
Once you’ve purchased your profile you will receive:

An online account to review your SmartHealthReport

A packet via the mail, which includes:

Driving instructions from your home to the closest
certified laboratory draw center

Draw instructions for the most accurate results

Your laboratory requisition form for your selected
labs. This is not an in home test kit. When you
order a test, we will set up an online account for
you to view your SmartHealthReportTM.
You may not order a test in New York or
New Jersey at this time due to State regulations.

Yes. MyHealthResults.com will help
educate you. Simply fax your lab results to us. Go to the products page and
select the Fax My Labs, complete the purchase and the fax number will be
provided with your confirmation number. MyHealthResults.com will provide you
with secure online access to a SmartHealthReport™ that will help educate you while you take charge or your health. It’s Your
